The growth timeline after a transplant, month by month

Why nothing seems to happen for two months, when new hair actually appears, and how long until you are looking at the final result.

The short version: the transplanted hairs shed within two to four weeks, then almost nothing visible happens for about two months, new growth starts somewhere around month three or four, and the result keeps thickening until somewhere between twelve and eighteen months.

Month one

Days one to three, the grafts settle and scabs form around each one. Then, usually in weeks two and three, the transplanted hairs fall out. This is shock loss and it catches people badly if nobody explained it. The hair shaft is what sheds. The follicle stays where it was placed, resting under the skin.

Months two and three, the quiet stretch

This is the hardest part psychologically, because the mirror shows nothing. The scalp can look the same as before the operation or even patchier. Some redness may linger on fairer skin. The donor area has usually closed by now. Underneath, the follicles are waking up on their own schedule and there is nothing to see yet.

Months three to twelve, the growth

  • Months 3-4: fine, wispy hairs come through. They look thin because they are new.
  • Months 5-6: those hairs thicken and darken. This is usually when other people start noticing.
  • Months 6-12: density fills in. Around months eight and nine most people are looking at roughly 70 to 80 per cent of the final result.

Year one and beyond

Full maturity lands between twelve and eighteen months. Larger sessions tend to need the whole eighteen. The last stretch is less about new hairs appearing and more about the ones you have getting thicker.

What you can and cannot speed up

You cannot make a follicle grow faster than its own cycle. No supplement, shampoo or serum changes that, and anyone promising otherwise is selling something. What you can do is remove the things that hold growth back: sleep, protein in the diet, keeping stress off the boil, treating the scalp gently, and staying consistent with whatever routine your clinic approved.
